In general, for the treatment of mild to moderate dental pain, the most appropriate options are:<br/><br/><br/>    * Acetaminophen (Tylenol), 650mg every 4-6 hour as needed for dental pain;<br/>    * Non-steroidal anti-inflammatory drugs, or NSAIDs (Advil, Motrin, Aleve), 400mg every 4-6 hours as needed for dental pain; and<br/>    * COX-2-selective inhibitors, celecoxib (Celebrex) and rofecoxib (Vioxx), 25-50mg per day as needed for dental pain.<br/><br/><br/><br/>Acetaminophen: Acetic acid and p-aminophenol, or APAP-commonly known as acetaminophen-is classified as a nonnarcotic pain reliever. Dentists generally use it for mild to moderate dental pain. It acts as both anti-pain and anti-fever. It has rapid painkiller action. Acetaminophen, unlike nonsteroidal anti-inflammatory drugs, or NSAIDs, has little anti-inflammatory action. It generally is safe for acute dental pain. At very high single doses it causes liver damages.<br/><br/>Acetaminophen's dose of 500-650 milligrams is good dental pain-reliever. However, such relief is brief, peaking one hour after administration. Significantly more dental pain relief can be provided by acetaminophen's dose of 1,000 mg. At this dosage, the maximal efficacy of acetaminophen is achieved and last up to four hours after administration. There is no more oral pain relief in dose higher than 1000 mg. <description><![CDATA[<p>Preventing Postoperative Sensitivity After Dental Fillings<br/>Preventing Postoperative Sensitivity After Dental Fillings<br/>Today, more patients ask their dentists about white fillings because they desire their teeth to appear natural when they laugh, talk and smile. White fillings, also called composite fillings, are made from tooth-colored materials that restore the natural appearance of a decayed or previously filled tooth.<br/><br/>A composite filling generally requires only one visit, during which the tooth is prepared and restored. A benefit of composite fillings, as compared with other dental restorations, is that they require less of the healthy part of a tooth to be took away to hold the filling in place. This is because of composite materials' ability to fuse to teeth adhesively.<br/><br/>Postoperative tooth sensitivity in composite restorations relentlessly continues to be a common, yet unpredictable, problem in dentistry. Such complication is even more unforgiving when the extreme sensitivities occur on teeth that were not sensitive before the restorations were placed. Despite meticulous use of dentin bonding agents, dentists and patients are faced with the sensitivity problem daily. Sometimes the dentist needs to rework these restorations. In rare instances, the dentist has to perform root canal therapy on the painful teeth.<br/><br/>Practitioners have gradually developed many preventive methods to overcome the sensitivity challenge. When postoperative tooth sensitivity is expected, combining two or more of the following &quot;desensitizing&quot; techniques may be necessary. Numerous combinations of the methods have been described and supported by clinicians. The combination technique that I have used with nearly 100 percent success follows:<br/><br/><br/>   1. Use of self-etching primer (such as Kuraray SE Bond, Kuraray America, New York City);<br/>   2. Applying tooth-desensitizing (GLUMA) solutions on the tooth before filling placement;<br/>   3. Laying a thin (about one-half millimeter-thick) resin-reinforced glass ionomer liner on deepest cavity surfaces; and<br/>   4. Inserting a sheet of resin-based &quot;flowable&quot; composite between the tooth and the final filling.<br/><br/><br/>Sensitivities can be also induced by other key events not related to composite placement. For examples:<br/><br/><br/>    * Accidentally burning the pulp during cavity preparation, especially when the remaining dentin is less than 1.5 millimeters thick and when there is inadequate water coolant sprayed during cavity preparation;<br/>    * Not removing all decay; and<br/>    * Bonding failure because of water and saliva contamination<br/><br/><br/>Composites are preferable for obvious cosmetic reasons. Your dentist may recommend another material or restorations, if the decayed area is large or is subject to heavy chewing pressure. Other types of white fillings include composite inlays and porcelain inlays and onlays. Inlays and onlays are used to restore teeth that are badly damaged by decay or wear. They may be placed on the chewing surfaces of the back teeth when aesthetics are of concern.<br/><br/>White fillings are more expensive than other dental materials. However, most patients find these natural-looking restorations well worth the extra cost. White fillings, like other dental materials, may need periodic replacement. If the edge of the filling eventually pulls away from the tooth, bacteria can get between the filling and the enamel and eventually may cause new decay underneath the existing filling. Few people may experience some sensitivity to hot and cold temperatures in the newly filled tooth for a few days or as long as a week. If the sensitivity continues beyond that time, please contact your dentist. Our patients are given the option of dental implants or a bridge to replace one or more missing teeth.<br/><br/>If a patient has bone loss throughout the mouth and has a lower denture that is loose, we recommend dental implants which can help keep the denture in place.<br/><br/>We like to inform our patients of all of their options and let them make an informed decision. We do all procedures in our office without the need to refer to a specialist.<br/><br/>Does an insurance company cover dental implants? About half of the insurance companies are paying for dental implants. We have discovered if they say a dental implant and related services are not a benefit, they consider paying as a bridge instead. We offer CareCredit in our office to help our patients get their dental treatment with monthly payments.<br/><br/>In order for us to make an accurate diagnosis, we must have a current x-ray of the area. If it is determined that our patient is a good candidate, we have them watch a video, we go over the consent form, and make financial arrangements.<br/><br/>The steps for a dental implant:<br/>-- Surgery: the doctor makes an incision in the gum tissue, to place the implant post directly into the bone. With the current x-rays that we have, he is able to determine which implant post is the correct one. He uses a drill to make an entrance site into the bone to place the implant. An x-ray is taken to ensure that the implant is placed correctly. Surgery is less traumatic than the surgery to remove the tooth. Dr. Joe will place sutures that need to be removed by him.<br/>-- A post op appointment and a suture removal appointment are made.<br/>-- Usually three months after the initial surgery, we schedule an appointment for another x-ray to determine if the bone is healing around the implant.<br/>-- If the patient is healing well we will schedule them to uncover the implant post and attach an abutment and take an impression soon thereafter for a crown to be placed on the implant post.<br/>-- The impression is sent to the lab for a custom made implant crown.<br/>-- Three weeks later, your missing tooth is replaced with the crown and your treatment is complete.<br/><br/>So what does all of that mean? The implant post is placed in the bone and the gum tissue and bone heal around it, (it is essentially the root of the tooth). The abutment is the attachment that is placed on top of the implant post. The crown is made and cemented to the abutment. Read my patients statements that I have heard in my 17 years of working in the dental field and my responses to help them understand why Preventive Dentistry is really important.<br/>Preventive Dentistry<br/>Enlarge Image<br/>PREVENTIVE DENTISTRY<br/><br/>Over the last 17 years, working in the dental field, it amazes me that people still do not understand the importance of preventive dentistry.<br/><br/>As an office manager, I speak to patients daily whether on the phone or in person. Every day I hear the same thing from our patients.<br/><br/>&quot;I don’t have insurance and can’t afford to come into the office every six months.&quot;<br/><br/>My response to this is… If you came in to our office on a preventive basis, we can evaluate the health of your teeth and gum tissue. We can find decay before it destroys more tooth structure.<br/><br/>&quot;I’m not going to schedule now, it doesn’t hurt.&quot;<br/><br/>My response to this is… It doesn’t hurt now and that is a good thing. The reason it doesn’t hurt now, is that the decay is only in the enamel, if not treated it will spread into a tooth structure called dentin. Dentin is not as hard as enamel, therefore the decay will become rampant. Once the decay is into the dentin, that’s when it will hurt and more than likely will end up needing a root-canal and crown.<br/><br/>&quot;If I need a root-canal and crown, I’ll just have it pulled.&quot;<br/><br/>My response to this is…. Keep in mind, when it is time to replace that missing tooth, with either a bridge or a dental implant in our office, your cost for replacing that tooth will be more expensive than a root-canal and crown.<br/><br/>&quot;I don’t need to replace a missing tooth.&quot;<br/><br/>My response… No you don’t need to replace a missing tooth, but let me tell you the consequences that may arise. First, your teeth will shift, causing your bite to be off. Second, with your bite being off, this may cause TMJ problems. Third, opposing teeth will super-errupt, exposing dentin. Remember dentin is not as dense as enamel, being more sensitive and more likely to get decay. Fourth, as we age into our senior years, we need our teeth to chew our food. You can minimize the side effects<br/><br/>As mentioned above, some people experience minor side effects from the tooth whitening process. These toothpastes contain ingredients that can help to soothe the nerve endings of your teeth. If the treatment involves a tray, wear it for shorter periods of time. It may take longer this way but you will end up with the same results.<br/><br/>5. You can get dental quality results at home<br/><br/>This may surprise you, but whether you use an at-home teeth whitening kit or go to the dentist you will generally end up with the same results. So why do people pay so much more to have their teeth professionally whitened by a dentist? Mostly because of the dramatic results they can see right away. At-home kits usually take a couple of weeks to get the results of one or two dental visits. The up-side for the kits is that they are much cheaper than a professional whitening done by the dentist.<br/><br/>6. Whitening doesn’t last forever<br/><br/>Generally, a teeth whitening will last for 6 months to a year. Of course this depends on the individual and the method of whitening that was used. For instance, an expensive dental office whitening will last longer than a cheaper method such as whitening strips. Many people feel that whitening kits with a custom mouth tray are the most cost effective way to keep teeth white because once you have the tray made you can simply purchase more gel at any time.<br/><br/>7. Whitening toothpastes have a misleading name<br/><br/>&quot;Whitening&quot; toothpastes, in most cases, do not contain peroxide which is the active ingredient for effective teeth whitening. Instead, they use mild abrasives and polishing agents to clean your teeth and help remove surface stains. Extra clean teeth can appear brighter, but whitening toothpastes will rarely whiten your teeth by more than a shade.<br/><br/>8. Watch out for the foods that stain<br/><br/>Heavily pigmented foods such as soy sauce, most varieties of berries, and cherries will stain teeth. Drinks such as coffee, colas and other dark soft drinks, teas and cranberry juice can also cause staining.<br/><br/>You can do a few things to keep these foods from staining your teeth such as brushing your teeth as soon as possible after eating staining foods. Don’t forget to use a straw for drinks that stain. The straw will keep the staining liquid from getting on the front teeth.<br/><br/>9. Brushing your teeth hard and often will not make your teeth whiter<br/><br/>Actually the opposite is true. Brushing too hard or using a hard bristled toothbrush can actually brush away the enamel of your teeth over time. The thinner your enamel gets, the more the second layer of the tooth shows through. Unfortunately the second layer, called the dentin layer, is a yellow or brownish color. Yellow showing through on your teeth is certainly not the desired result. So, if you want to keep your enamel, brush gently and with a soft bristled brush.<br/><br/>10. Watch out for dangerous &quot;natural tooth whiteners&quot;<br/><br/>There is a lot of bad information out there about teeth whitening. But there is definitely one health related subject I would like to have known more about and adhered to: early childhood hygiene practices.<br/><br/>I had problems with my teeth from the minute go. I had to have fillings, root canal work, tooth extractions. You name it, I had it. Even eating all those taffy apples and other candy (especially at Halloween) would have been alright if only I had known about and practiced proper (and I mean proper) gum and health hygiene. Call it hindsight.<br/><br/>Ironically, at this stage in my life I frantically take care of the few teeth I can still call my own. I brush my teeth (and gums) as soon as I get up in the morning and after every meal. I use mouthwash regularly. I go for biannual dental checkups. I make it a point to get to a dentist as soon as I suspect the slightest dental problem.<br/><br/>In our day and age anything can happen. My next statement may sound a little bizarre - like something out of science fiction - but so did reaching the moon fifty years ago. I visualize people in this lifetime cutting a third set of teeth. We cannot avoid bacteria in the air. When we breathe it enters the mouth and attaches itself to everything, including the teeth. In fact, there are more microcosms in one mouth than there are people on the earth.<br/><br/>Sugar is the main culprit in tooth decay, because that is what the bacteria eat. Bacteria then produce acids as a byproduct. Those acids eat at the enamel of our teeth, until hole or cavity in the tooth appears.<br/><br/>Our story begins in the 17th century, when sugar plantations developed in the &quot;new world.&quot; Until then food was produced and prepared with much less added sugar. Then the 18th century saw sugar beets being harvested in England. Now, virtually everything we consume, from cereal in the morning to steak and eggs at night contains extra sugar. Bacteria on our teeth count themselves lucky to live in the 21st century, where there is an almost unlimited supply of free food for them to thrive on.<br/><br/>We are in cahoots with sugar and bacteria when we do not brush and clean our teeth. Leaving the bacteria to feed upon sugar and produce acids in our mouths allows the bacteria time to form a visibly organized colony between the gums and the tooth that we call plaque. Plaque actually acts as a cover for the acids that sit on the surface of our enamel. Without cleaning, acids will eat at out enamel almost at will, creating tooth decay and dental caries.<br/><br/>Decay is demineralization. In other words, the outer tissue of the tooth is so hard because it is 95% mineral. The inner tissue of the tooth, dentin, is a little softer because it is only 66% mineral. Normally, saliva is a natural remineralizer when acids have begun demineralizing, but when plaque is involved saliva is almost powerless to repair the damage. Acids will begin with a little hole in the enamel, and once it makes it through to the dentin, it eats the tooth tissue from the inside out. This means serious dental action: fillings, or even a root canal.<br/><br/>With plaque the acid concentration is also higher (Ph 4 or lower), packing a more potent punch through the outer enamel tissue of the tooth. Saliva could take two or more hours to even penetrate the plaque and begin the healing process.<br/><br/>There are a variety of preventative measures to take. I know people who have taken to a no-sweets diet to cut down on their sugar intake. There are many who bring toothpaste and a toothbrush to school or work with them in order to clean after each time they eat. Many more carry floss with them.<br/><br/>May I caution against two things? Please do not depend too heavily on fluoride. Little children who take in too much fluoride, even by swallowing toothpaste accidentally or unwittingly, develop dental flourosis, or yellow and white stains on their teeth in later childhood. Take it in healthy doses.<br/><br/>The second caution is to avoid too much brushing. People who brush excessively or applying too much pressure tear away the gums and expose the roots directly to the acids.<br/><br/>Now, may I suggest two things? Clean frequently and softly by brushing and by flossing. If you can hear the brushing sound as much as the other noise around the house, you are brushing too loudly. With brushing and floss, you don’t need to try too hard to eliminate the plaque or the bacteria.<br/><br/>My next suggestion is to carry around gum that is not only sugarless but also supplemented by a natural element called xylitol. Be careful that you choose a gum where xylitol is the leading ingredient. Xylitol fights against the habit bacteria has of settling into tissue to live. Xylitol is a natural bouncer, making cleaning throughout the day easier and cleaning in the morning or at night more thorough.<br/><br/>One transcendent element is sugar. It is almost impossible to escape, and it is not healthy to escape altogether. <title><![CDATA[No more fillings?]]></title>
<description><![CDATA[<p>No more fillings?<br/>It's one of life's horrors. But a controversial new treatment could soon make the dentist's drill a thing of the past. Leo Hickman reports.<br/><br/>The nervy flick through Woman's Realm in the waiting room... the white-knuckle grip on the plastic chair as you are lowered to your fate... the high-pitched scream of the drill. &quot;Please open your mouth,&quot; says the dentist. &quot;Just try to relax.&quot; Having a filling must rank as one of the most unpleasant experiences of childhood and it doesn't get any better as you get older; in fact, generally the fillings get worse and so does the drilling.<br/><br/>Over the past 30 years, our teeth have got much better. We look after them better, and we're all round more tooth-aware. Many people under 30 can now boast that they've never even had a filling or a crown - something almost unheard of a generation ago. But for those people who don't have the perfect teeth of the fluoride generation and still get cavities, drilling remains a vile and depressing risk every time you go for a check-up.<br/><br/>Now, along comes a new treatment that promises to put an end to this painful collective experience. It's not yet widely available, but has been introduced at a handful of private dental practices across the country. The treatment, called HealOzone, could, according to its cheerleaders, finally banish unsightly fillings once and for all from our mouths.<br/><br/>At first glance, the treatment looks too good to be true. It is, we are told, fast, cheap and almost 100% effective. First, the treatment involves using a laser to detect tooth decay. Then a special airtight rubber cup is attached to the tooth. The air is sucked out of the infected cavity, and replaced with ozone, a powerful germicide. The gas kills the offending bacteria almost immediately, and the cavity in the tooth then either &quot;remineralises&quot; naturally, aided by calcium salts in your saliva, or, if the hole is too big, is filled with a cosmetic white filling (but with no drilling involved).<br/><br/>James Hull Associates, currently the largest chain of dental practices to offer HealOzone, has the required equipment at three out of its 40 practices. Dr Peter Murray, the company's clinical director of dentistry, is bullish after seeing the results. &quot;I see nothing but good coming from it. It works in 90% of cases first time, with the remaining 10% being treated successfully upon second treatment. The treatment takes about 40 seconds per tooth. Beforehand, if you were doing good quality fillings on a back tooth, you were taking at least half an hour with injections and so on.&quot;<br/><br/>Advertised as pain-free, HealOzone could actually work out cheaper than conventional fillings. &quot;We've set a rate of £45 a tooth with an initial check-up cost of £65,&quot; says Murray. &quot;A conventional white filling costs anywhere between £65 and £95 per tooth.&quot; But Murray also believes there needs to be a major change in conventional thinking before it is widely available outside private practices. There's also the small matter of the equipment currently costing £15,000.<br/><br/>But with high-profile industry fans such as Dr Julian Holmes of Wokingham's Adentec Clinic claiming that dentists can reap £576 per hour in profit using HealOzone compared with £108 with traditional fillings, that could act as a spur by dentists to invest.<br/><br/>What could slow things down, however, is initial scepticism about the use of ozone by bodies such as the British Dental Association. &quot;Ozone is a very interesting element,&quot; says Gordon Watkins, chairman of its health and science committee. &quot;In the atmosphere it is very beneficial to us, but down at ground level where we breathe the stuff it is known as a pollutant and major cause of smogs and respiratory problems.<br/><br/>&quot;The use of ozone in medical care extends back to before the first world war. It was used to treat gangrene and anal problems. Basically, they would blast it up your back passage. Nowadays, it doesn't have any other application in healthcare in this country. But, interestingly, it is used extensively in Cuba. Apparently, you can't go into a casualty unit in Cuba without being sprayed with ozone beforehand.&quot;<br/><br/>While Murray counters that HealOzone is &quot;completely safe because each tooth needs an airtight seal before the ozone is inputted and is so localised that there's no problem&quot;, Watkins adds that the US authorities are also sceptical about the medical use of ozone. The federal food and drugs administration is adamant: &quot;Ozone is a toxic gas with no known useful medical application in specific, adjunctive or preventive therapy. In order for ozone to be effective as a germicide, it must be present in a concentration far greater than that which can be safely tolerated by man and animals.&quot;<br/><br/>&quot;We would certainly like to see more research done on it,&quot; says Watkins, rather diplomatically. &quot;We want assurances that ozone is not harming the mouth.&quot; He stresses, however, that the debate over ozone use is largely a side issue when it comes to dentistry. The good news, he says, is that the majority of a dentist's work is now preventative or cosmetic - a marked difference from the fillings, bridge-work and extractions that took up the dentist's day a generation ago.<br/><br/>&quot;The dental condition of the population has improved dramatically over the last 30 years,&quot; he says. &quot;We now know the dietary components that cause tooth decay and we know how to strengthen the teeth with the use of fluoride in toothpaste and in our tap water - something that has been taking place since the 1960s. Dental decay is largely a social disease, namely, the culture of high sugar consumption. Dentists are increasingly just treating gum disease - the world's most prevalent disease - and offering hygiene and diet advice.<br/><br/>&quot;The balance is certainly changing. I'm in my 50s and have eight fillings which is actually quite low for my generation. You can also chew whole cloves for some pain relief as well.<br/><br/>Activated charcoal, mixed with water to make a paste, and applied to gauze, can really help if you bite down gently on the gauze. You can buy activated charcoal at many health food stores.<br/><br/>Destroy the Bacteria<br/>Chewing a clove of garlic with a little bit of rock salt can help destroy the bacteria which causes tooth decay, slowing down the action of the decay and giving you a little relief. You may also be able to use a small piece of onion as a home remedy for a toothache by placing it against the bad tooth.<br/><br/>Painful Cavities?<br/>Pepper mixed with clove oil can help alleviate a toothache caused by cavities, as well as being an excellent substance to brush your teeth with when mixed with salt.<br/><br/>Bay berry bark can be powdered and mixed with vinegar. Then it can be applied to the gums for toothache relief.<br/><br/>Asafoetida, a strong smelling spice used in Indian cooking, has been traditionally used for toothache. Grind it in lemon juice and warm the mixture, then soak a cotton swab in it. Applied directly to the tooth or gum around the tooth, it can help to relieve the pain.<br/><br/>Remember, if you have an unexplained toothache, go to your dentist immediately. Dental cavities usually do not get better on their own. Oral health and general health should not be interpreted as separate entities.&quot;<br/><br/>We know now that cavities and gum disease are bacterial infections that can be prevented. An open wound such as bleeding gums, is a perfect source for specific bacteria, hiding between the teeth and gums, to enter the blood stream and make their way to major organs to begin new infections.<br/><br/>So, you can see the importance of a very thorough oral checkup. It isn’t just about having a pretty smile or even chewing those steak bones into old age. It is about detecting other problems and ultimately having the healthiest body that we can possibly have.<br/><br/>Beyond the Basics:<br/><br/>1. A thorough review of your medical history including any new medications, hospitalizations and other health concerns.<br/>2. In addition to the obvious — your teeth — a soft tissue exam should be performed that includes your cheeks, your tongue, your palates, (hard and soft) and your throat.<br/>3. An assessment of your gums (color, texture, contour, bleeding, pus) and probing of periodontal pockets.<br/>4. An external exam of your neck and palpation of your glands and lymph nodes.<br/>5. A visual exam of your head, neck, lips and face for any crusty, discolored or lingering sore that has been there for 2 weeks or longer.<br/>6. Checking for a change in your bite, or any obvious clicking when you open your mouth wide.<br/>7. An assessment of your oral hygiene effectiveness and a review of techniques that will help you improve.<br/>8. And if you are lucky enough to be going to a dentist who has a microscope, he/she will evaluate bacteria levels in your mouth that not only threaten your teeth and gums but also your full body health.<br/><br/>It is in our best interest to see our dental professional several times a year. It is the responsibility of our dentist and hygienist to check our teeth and restore the luster in our smile, but it is also our responsibility to make sure we get a thorough checkup. Besides the obvious — inside our mouth — all the areas of our face, head and neck need to be examined to make sure there is nothing lurking that threatens our long-term health.<br/><br/>If your dentist is taking short cuts, show him or her the checklist above and ask for a more complete exam. If you get an objection or a blank stare, then run to the nearest phone book or ask a friend about their dentist and find someone who will go beyond the basics. This helps in boosting self-esteem and carries with it many significant dental health benefits as well. This is because the gaps left by missing teeth can and do have an adverse effect on the way the rest of the teeth meet in the mouth. Such gaps can cause the remaining teeth to tilt and drift into them. Moreover, food particles are often trapped in the spaces and this increases the risk of decay and gum disease.<br/><br/>It is advisable to discuss false teeth options with an orthodontist before having any teeth taken out. One can, of course, choose to do nothing and avoid false teeth altogether. One can also choose to use dentures, which are false teeth sets consisting of removable plastic or metal plates fitted with artificial teeth. Yet another option is using bridges, which are artificial teeth anchored to adjacent natural teeth. Finally, one can choose implants, which effectively are metal ‘pegs’ attached to the jawbone onto which dentures or bridges are clipped or screwed.<br/><br/>One of the most commonly used options is dentures, which a dentist can fit a patient for immediately after tooth/teeth extraction. This is the only immediately available option then, because it takes several months for the bone and gums to reform stably after tooth extraction. A temporary plastic denture will not stay in place for over a few months, since the healing tissues shrink. Complete healing usually takes place in six months, after which a dentist can start making a long-term denture or bridge.<br/><br/>One can also opt for partial dentures, which are useful for the replacement of one or more missing teeth. A patient must adopt a routine of removing such dentures for cleaning, and having them out of the mouth out at night. Partial dentures come in various types. However, they usually comprise of a metal or plastic plate (or a combination of both) that holds plastic or porcelain false teeth. They usually incorporate metal clasps to keep the denture in position. It is often possible to conceal them so that they are not visible while smiling and talking.<br/><br/>In cases where no teeth at all remain in the upper or lower jaw, full dentures are often the only option. A user needs to remove such dentures for cleaning and take them out altogether before sleeping at night. Full dentures invariably comprise of a plastic base carrying plastic or porcelain teeth. Full dentures for the upper jaw cover the roof of the mouth, with a very thin layer of saliva between the roof of the mouth and the denture providing suction to keep them firmly in position. They often interfere with people’s perceptions of taste and temperature because they cover the palate.<br/><br/>Retaining a full denture for the lower jaw in place is not that simple. The floor of the mouth features complex musculature whose working can push the denture out of place. Over the time, however the tongue muscles usually adapt to the shape of such a denture and keeping it in place becomes easier.<br/><br/>Bridges are useful in replacing one or two teeth, providing that suitable teeth on either side of the gap exist to help anchor false teeth into place. <description><![CDATA[<p>Common Medications for Dental Pain (Part 2)<br>The pathophysiology of dental pain is a complex system. The use of combination analgesics that act at multiple pain sites can improve dental pain relief. (Part 2)<br>Common Medications for Dental Pain (Part 2)<br>In part 1, nonnarcotic pain medications for dental pain are discussed. Dentists use these medications for relieving mild to moderate oral pain. This article describes the narcotic medications. For moderate to severe dental pain, the typical medications are:<br><br><br><br>&nbsp;&nbsp;&nbsp; * Tramadol (Ultram), 50mg every 6 hours as needed for pain.<br>&nbsp;&nbsp;&nbsp; * Tramadol with acetaminophen (Ultracet, containing 37.5 mg tramadol hydrochloride and 325 mg acetaminophen), one table every 6 hours as needed for pain.<br>&nbsp;&nbsp;&nbsp; * Acetaminophen (Tylenol#4, containing 60 mg Codeine Phosphate and 300 mg Acetaminophen), one table every 4-6 hours as needed for pain.<br><br><br><br>For severe pain, opioid combinations are advised. For example, one Vicodin ES (10 mg hydrocodone and 750 mg acetaminophen), can be taken every 4-6 hours as needed for pain.<br><br>Opioids: Opioids are narcotic agents that act on the central nervous system. Side effects-including nausea, constipation, dizziness, sedation and respiratory depression-are common with opioid therapy. However, the relative risk of opioidlike side effects varies.<br><br>Although opioids as a class are effective dental pain reliever, some commonly used formulas show poor efficacy for dental pain. Other drugs with fewer severe side effects can have similar results. For examples, codeine alone has not been found as effective as other common analgesics (acetaminophen and NSAIDs) for relief of dental pain. Oxycodone, hydrocodone and propoxyphene are about as effective as codeine. Dihydrocodeine, penta-zocine and meperidine show no advantages over codeine orally and can even be less effective. Their effectiveness in combination therapy (combining opiods with acetaminophen and NSADs) is better than that in monotherapy.<br><br>Tramadol: Tramadol is a synthetic, centrally acting pain reliever. It is indicated for moderate to moderately severe oral pain. Its analgesic action affects both opioid receptor and serotonin uptake. This suggests that tramadol's effect is not mainly through narcotic mechanism. Tramadol, thus, is a nonscheduled drug. The serious side effects typically associated with opioids-such as dependence, sedation, respiratory depression and constipation-occur less often with this medication. Tramadol also has a low rate of abuse, about one per 100,000 persons. The side effects commonly seen with tramadol include nausea, dizziness, drowsiness and tiredness.<br><br>Tramadol's lack of sedation is particularly important for same-day dental surgery. Tramadol does not have the same side-effects like NSAIDs or traditional opioids. Adverse side effects generally are mild and transient. Importantly, tramadol does not have the ceiling dose effect common to many other analgesics. Recent studies show that tramadol is a good postsurgical and dental pain killer. They also show that tramadol has a dose-response effect. For instance, in one study they reviewed, tramadol 200 mg was more effective than 100 mg after third-molar extraction. Unlike aspirin and acetaminophen with codeine, which have an analgesic duration of about four hours, tramadol provides analgesia for five to six hours after dental surgery.<br><br>Benzodiazepines are increasingly being used to decrease patient's anxiety. Their sedative, "anxiety-reducing" and "forgetful" properties, along with their mild respiratory depression, are especially helpful for lowering the "view" of dental pain. By reducing the dental fear, the patient becomes less sensitive to painful stimuli.<br><br>Midazolam: Midazolam has the ability to decrease postoperative anxiety. It provides complete surgical amnesia (memory blockage) that lasts about 25 minutes. A multidrug combination of fentanyl, midazolam and metho-hexital (commonly used in intraveous sedation for wisdom teeth removals) gives better pain control but produced deeper sedation.<br><br>Treatment of anxiety related to dental procedures is most worthwhile for children. Extreme preoperative apprehension may need more anesthesia and lead to postoperative negative effects. Oral midazolam has been shown to produce significant amnesia in children when it is given10 minutes before a surgical procedure. Recent clinical trial of oral tramadol mixed with midazolam provides effective pain relief during and after surgical procedures for kids.<br><br>Diazepam: Diazepam is another useful benzodiazepine that treats oral pain associated with muscle spasm. However, its use is limited by long-term sedation, abuse potential and dependence potential. Diazepam may have additive side effects with other central nervous system depressants. Combinations of benzodiazepine and opioids are used widely for conscious sedation but are associated with significant risks. Over time most teeth typically lose their luster and brightness because they accumulate stains made by foods and beverages. New whitening agents can take away these stains without harming your teeth.<br/><br/>There are two classes of dental stains. For intrinsic stain, the teeth's discoloration is caused from the &quot;inside&quot; by &quot;internal&quot; agents such tetracycline and fluoride. In contrast, in extrinsic stain, the teeth's discoloration is caused from the &quot;outside&quot; by &quot;external&quot; agents such as tea, coffee, and tobacco. Intrinsic stains are harder to remove than extrinsic stain. I recommend bonding and veneers treatment for intrinsic stains and bleaching for extrinsic stain.<br/><br/>By far the best and fastest bleaching solutions are offered by your dentist. During a typical whitening procedure, your dentist will fabricate a custom tray to hold whitening gel against your teeth. You take the trays home and wear the gel-filled trays each night or for several hours during the day for several weeks.<br/><br/>Dentists can also bleach teeth in the office. This is called power bleaching or laser bleaching. Both techniques work equally well and are offered both as a &quot;jump start&quot; prior to home whitening and as a separate &quot;stand alone&quot; procedure.<br/><br/>While you can get over-the-counter whitening kits, they usually aren't as effective as professional whitening procedures. Because the over-the-counter trays are not custom fitted, the whitening agent may leak out and irritate your gums. Want a whiter smile? Ask your dentist about dental whitening.<br/><br/>Besides, you need a dental evaluation prior to whitening your teeth for several reasons:<br/><br/><br/>    * The teeth to be safely bleached if it has no cavity on it.<br/>    * There are two type of stains on the teeth: extrinsic and intrinsic. Extrinsic staining is the stain on the teeth that comes from ther outside such as tea, coffee, and smoking. Intrisinc staining originates from the inside of your own body such as tetracycline and blood. The method and the recommendation to whiten your teeth differ for each situation.<br/>    * You will need someone to monitor your progress. Yes, there is such thing as OVER WHITENING!<br/>    * Most of the over-the-counter product will lighten your teeth a little at a higher risk of swallowing a lot of the bleaching chemical.<br/>    * o inrease the shelf-life, most of the over-the-counter whitening products are added with an acidic preservatives. The acid dissolves the precious enamel of the tooth and causes extreme tooth sensitivity. Professional products have no preservable; the only way to prolong the shelf life for the professional products is to refrigerate them.<br/><br/>      The best way to whiten your teeth without hurting them is to see your dentist. The whitening toothpastes that are on the market are abrasive to your teeth and could cause damage to the enamel. The ADA (American Dental Association) has not approved any over-the-counter whitening products such as Crest White Strips.
